Two good reasons. First, the design makes them a little more endearing and it's a constant reminder these are little kids (12 is little) who are vulnerable. The impact of Riko being poisoned and having her arm broken and nearly amputated is absolutely horrifying specifically because of their design. Second, is to highlight the contrast between humans (frail, impermanent) and the abyss (menacing, eternal). That aside, yes the penis jokes were a little much.
