Gargan was nearly MIA for decades; he'd just randomly pop up in a few different character's books (but rarely Spider-Man fot some reason) because writers would want a pre-established villain to act in a 'bruiser' role. His mental state would vary wildly, as would his connection to his costume (sometimes he was literally trapped in it, sometimes he wasn't).
