I think a lot of people straight up aren't aware that the "hulking nurse who looks like a man" bit is a joke that was pulled straight from the post-WW2 landscape of hospitals where a lot of nurses genuinely were fucking straight up YOLKED because being a nurse back then was tougher and more violent than it is today. And even these days, a lot of nurses are quite physically fit or strongfat.
That's why the juxtaposition is with the sexy nurse visual: the sexy nurse was something more or less invented by wartime propaganda, porn, and mass media. The whole joke relies on the characters not actually having much experience with nurses or ones that aren't just glorified assistants/secretaries.
