The short answer is it's autism and a lack of self awareness. Nerds aren't the most well adjusted people especially when they have autism. Hell, we could even pull an example in this very thread. If you asked "who likes seeing a woman wet herself" almost everyone here would likely say yes, but if you asked "who likes seeing a woman wet herself while wearing a diaper" I guarantee a good portion of them who just said yes to the first question will suddenly be disgusted, but not really be able to give a good reason for why it's suddenly disgusting now. One of the most common answers I have seen when the topic has naturally come up however is them trying to loop diapers in with scat. However if you're looking for a trrpg that is more likely to welcome omo stuff I'd imagine the goblin slayer ttrpg would be a lot more welcoming to the idea since girls pissing themselves is a staple in the series.
