Very firmly. You'll probably rip more than skin if tried to rip it out, as long as you get correct size.
I was only worried about it getting stuck from being absorbed into skin during long term play, but it never happened. During that time I'd slide it around to prevent such. But I think this would be unlikely.
The Tetherspout website indicates taht some people are worried about losing the rod in the bladder, but he said he's never heard it happening.

Yes. The only insecurity is the lock you use on the tether - I've been tempted to get a rod with c-ring grooves milled in them and insert these in it (https://www.sowatool.com/CA/p/146477?srsltid=AfmBOooRFjX6RctUiBleIO4hWvf1LQLz2_qWB_8tiGwZHV2TmYb9VjPv), but I have not done so...yet... I want to get a better fitting cage first.
