To add a lot and make ot look realistic, you'd have to wither shrink the glans or expand the foreskin so it looks as if the smegma is packed beneath the foreskin making it raised. The easier option, if you want to add a lot of smegma, would be just to use a retracted foreskin

I think just colouring like Zyldark (yellows) or Milk Flaker(golds and browns) uses would work

I think that's only a feature in Photoshop, but it's odd since it is a fairly easy feature to implement. I wonder if it's copyrighted, so other programs can't do it since I've never seen any other let you keep the image based on the original until you rasterise it.
