It'd be entirely possible to write characters making good decisions and the villain(s) either reacting in equally smart ways. being so powerful that the good decisions don't matter, or external factors messing things up.

Find gun. Shoot slasher. Once slasher is on the ground, shoot slasher in the head until gun is empty. Good decision that would let you survive 99% of slasher films. Whoops, there was a second slasher that couldn't've been known about and now the ammo supply has been exhausted. Bam! Good decision, peril remains.
