They do something retarded called email reputation and it ultimately depends on whether or not your server's IP is in their DB for any reason whatsoever. I haven't had any problems with self hosted email, and I was even able to use it to sign up to a few privacy violators during a small curiosity test run, so I either got really lucky and my email isn't on the naughty list by default, or I'm simply not immediately trying to go out and use it for all sorts of things asides from communicating with like-minded individuals with it.
