You ban the /64. Banning the individual IP makes no sense indeed.
Your banlist becomes an unsigned 64 bit integer. You don't care about half the IP.
And you can run dual-stack with no effort, because the 32 bit IPv4s can still be mapped within it.

Meanwhile what do you do with a spammer on CGNAT who shares the same IP as a hundred legit users? What do you do with a spammer who rerolls IPs with airplane mode on his phone?
