You don't. At best you can prompt set pieces to fill in a scene but keep it simple and consider 'blurry background' to give the illusion shit's happening back there that isn't an AI thrown together nightmare.

Probably a mainstay in the data of whatever artists you're prompting. It's just a bias many sexooo prompts will try to slip in there. Prompt each artist individually and see which one is responsible then adjust the weights.
