seems that it had a tenon that broke off & stuck into the mortise, and to "fix" it they just glued it on & probably drilled out the stem in turn

the way you'd fix that is by drilling that stuck tenon out, then re-adding a tenon to the stem
doesn't actually seem that hard, but if you don't have the right tools, you won't be able to do it

i might be all wrong about it though
it could be one of those weird pipes that use a reverse mortise & tenon setup
first step to fixing it would just to give it a good rinse with some warm water, its pretty dirty
