They basically have a mandated percentage of cases they have to reject, so essentially its a lottery as to whether you'll get your paid for or not. Generally if you need an expensive treatment of surgery then suddenly the lawyers will be trotted out to dick you over, they're not going to waste their time fighting stitches since thats a fairly minor procedure.
