I'm not that far in yet to experience it firsthand but from what npcs tell me it's not game over
You can lose stats and depending on what you gave birth to(human/beast/demon) there's different routes
Like giving birth to a human causes you to lose your powers, not sure where you go from there but the underground/slum segment of the capital city has a bunch of previously magical girls working as whores
Giving birth to a demon causes you to turn into one if your karma is high enough apparently
