Yes a cave man can die in his sin since the law of God is written on the hearts of men so he would naturally have an understanding of what good and evil acts are. Feeding his family would be an act of good and killing his wife and children would be an act of evil. Cave man doesn't need Holy Scripture to tell him what is right or wrong because again, the law of God is naturally already written on the hearts of men.
