It's the general feeling of the picture that make people assume that: taken in winter, in a black coat, very pale, the smile (while not forced) appears to lack energy. In fact, the smile adds to the sense of despair due to the usual almost cliched worldwide understanding of "smiles hiding pains". Her photo and one other of a woman giving a deadpan stare into the camera I think truly give off depressing vibes. Listen to this man. Depression is a self-pity trip and trying to help is going to siphon all your energy away and destroy you when you realise you've failed or, at worse, fed the self-pity further.
