Why aren't DNA exams as widespread as blood exams?
I would like to go to an actual physical clinic instead of taking my own blood samples and sending them via mail.

blood is useful for checking your health
dna is used for genetics and not an everyday thing

Because there's much less relevant information in your DNA than there is in your blood and it's easier to figure the relevant bits out of your blood than it is to figure it out from your DNA. Like a machine that weights few grams can near instantly produce you a result that is immediately useful to you if you are suffering from diabetes with a drop of your blood or you can go to a blood test and immediately figure out that "yes you have disease X here's the prescription that will fix it the apothecary is by the door" or what ever. Meanwhile genes only predispose you towards certain things, they may tell you that you are 10 times as likely as an average person to get a cancer but you may live your life without getting one or you could do your best to avoid the predisposed vulnerability to cardiovascular disease with healthy food and lifestyle and still die from your arteries being clogged up, in either of these cases the DNA information gave practically speaking 0 value. And that's again without saying the fact that a DNA test is much more complex and expensive procedure to perform and the information it can give is very questionable, most of it is just correlation based (people with this gene are Y times more likely to develop Z) instead of actual causation based hard information.

Yes. And you may be tested for these if they have reason to believe you have them.
But in most cases, whatever is wrong with you is not something they'll be able to glean from genetic testing.

99% of medicine is about casting as broad a net as possible to catch the most likely problems because that will help accurately diagnose and treat the vast majority of people.

It has to do with the basic medical patterns of getting diagnoses, prescriptions, and procedures, as well as justifying all of those things through bureaucracy and statistics.

Therefore, a DNA test needs to be issued in a medical context for some specific reason, like breast cancer risk, and not just because the patient feels curious.

Still, in the US you're free to pay for your own blood tests and DNA tests through any company who offers it, so there's no real problem there.
