There is and should not be any problem with either of these. If people are incapable of seeing the difference between a fictional world and our real one, then they are the problem.
Even if we take gnomes and give them attributes that correspond to Jewish stereotypes for instance, it should be a trivial thing for the player to understand what they are seeing in the game has nothing to do with the real world. Same with orcs, if we depict them as jungle dwelling savages, there is no reason to assume the game's developers hold such views of jungle dwelling cultures in the real world.
Of course genuine idiots will see that and make hurr durr assumptions, but the worse are people who see this, immediately take offense for some other culture because they know you can't publicly defend against something like this.
The world would be a simpler place if people stopped being intentionally retarded, but that is a big ask I suppose.
