I mean, it would have to be a completely different game. The twist of Postal 1 is that the Postal Guy didn't kill anyone. All residents of Paradise City were killed by a mysterious demon that simply possessed Postal Guy's body because his mind was weak enough to control. The reason why at the end of the game Postal Guy was put into the Asylum instead of a prison was because even though he didn't technicly kill anyone, he was still partly responsible for the tragedies throughout the city because he was the only one weak enough to be possessed by a demon in the first place and also had all the memories of the demon doing the killings that everyone blames him for.
I think if they wanted to create a direct sequel with the same MC, they would have to go the American McGee’s Alice route and make it take place inside Postal Guy's head, where he's going throughout a hellscape version of the events in Paradise City (basically imagine the whole game looking like the loading screen artworks in Postal 1) fighting demons and trying to reclaim his sanity in the process.
