With Infernal support, the Cathars last a lot longer, and then maybe the Antipopes in Avignon open a Hellgate. After that gradual Hell-pirate raids continue during the Ottoman/Barbary Corsair wars with Europe ends with them seizing Sicily as a forward base, followed by raids on the Spanish Levante against the weak Taifa kingdoms before they capture Gibraltar. Around the time the Hordes of Hell are invading the Balkans by land, they would invade Moldavia by sea. Dracula throws them out of the Balkans, but they hold onto Moldavia. Alternatively they could've seized it in the Crimean Wars from the weakened Ottomans and Russians. The power of Hell waxes and wanes alongside more worldly, mundane wars and the Wars of Religion. Finally the Napoleonic Wars weaken European states enough for the Hordes of Hell to successfully invade the Rhine Confederation and Poland.
As the Great War breaks out in 1914, the Hordes of Hell quickly seize the Low Countries but are unable to break through the Maginot Line or push into the HRE, so they entrench themselves.
We end up with a third of Europa united as the Apostate Axis, but although the slaughter and horror they inflict is unrivalled, they are in a precarious situation, their supply lines between the Hellish states thin and vulnerable
