By "the guy", I assume, you mean Skull, the Duty commander, because I don't think the quest should fail if someone other than you kills Lukash. If so, then it's meant to be hard to keep him alive in the assault, as it's supposed to be a reckless suicide mission, but the quest shouldn't fail if he dies.
As it's supposed to work, if Skull dies, some other guy from his squad takes command and you can turn in the quest to him, when the fight is over, as long as there's at least one Dutier left alive. It can bug out though.

Yeah, that can happen. For some reason, the Barrier quest completes before all the Monolithians are dead, so Cap can still die before you speak to him. It can also fail if you accidentally shoot any of the Freedomers and make them hostile or if you walk too far away from the Barrier during or after the fight.
If you're having a hard time with the Monolith, you should get a Vintar from one of the barracks at the Freedom base, or any other sniper rifle for that mission, makes the quest much easier.
