Some of the best "diaper" stories I've ever read were just good stories, with the added bonus of one or more characters being diapered. There were fappable sections in them, but a plot outside of "I need an excuse to diaper people" and having developed characters made me want to continue reading after nutting.

Write a good noir first, then incorporate the diapers.
