Not instantly, no, but even in the best case scenario I think that would have left the US in a case where both sides (or at least the more radical elements of both parties) would feel that political violence was just 'part of the system', despite the constant pleas from more mainstream colleagues to stop it.
You'd get to the point where even low level city government politicians were seen as 'fair game' for everything from marksmen to drive-by's, to knife attacks, to bombers, pretty quickly. Politicians all over the country would pretty much have no choice but to live under the management of their security personnel, possibly organized at the party level, as the SS showed that they couldn't protect a former President and current candidate. Sure the Secret Service ended up looking retarded and incompetent when the shots missed, but if they'd hit I have to assume that their reputation would have been annihilated pretty much instantly - you'd probably see them being either replaced or 'supplemented' (i.e. supervised) by some kind of PMC almost immediately (which would just be a whole other can of worms).
>tl;dr, it probably wouldn't have lead to Civil War, but the tone of American politics would have been changed for the foreseeable future, and would look more and more like some kind of even worse version of Northern Ireland at the height of 'The Troubles'.
