Tell him you cant walk properly and you use crutches, have anxiety, depression, autism, ADHD, you shit yourself and can't control your bowels, you have addictions to alcohol, drugs, you attack people if you're in public etc. Literally just say you're overwhelmed and need help and appointments to get help and meds (don't take any of the meds, it's just so you can use as evidence when applying for PIP)
>get someone to help you with PIP form like a charity or something
>look up PIP scoring system
>if your application is unsuccessful (usually it is) ask for a mandatory reconsideration
>if your application is rejected again (usually it is), apply for tribunal
>at tribunal stage your pretty much guaranteed to get what you want. You don't even need to turn up in person as you can just sit on the phone
>if that fails (highly unlikely) then you can go for upper tribunal
There's lots of info online the UK government by law has to share. The more evidence you gather from your doctor about health issues the easier it is.
