She went to the True Lab to prepare things for telling the truth and letting them out of the lab, the note was basically a "if I kill myself check the elevator"
Alphys did a lot of fucked up stuff but the date with her ends on Undyne saying that for them to have a chance at a relationship she needs to be honest about everything so she goes to do that which I think is fair, although not having a scene where Undyne reacts to everything is pretty underwhelming even though it would've probably messed with the pacing and mood since you go straight from True Lab to Asriel
