This card originally only had 1 evolved form, but they later added 2 different forms (one at a time) that you could choose from when evolving. The "new" forms only existed as evolved versions of the base form and not as a separate card of their own. This is the only card that works in this manner.
Most of the time, you just chose Blast Form, but Defense Form got gigabuffed later down the line
