It is a brain computer interface with AI at the helm and other classified technology that assists in the "simulation". This AI will silently watch you for a while, concoct a trauma/fear based simulation, and then carry out that simulation when the right conditions are met.

Depending on where you reside, if you're religious, if you believe in the paranormal/aliens, determines what kind of simulation you get. The only purpose of the simulation is weaponized mental illness. It will simulate something like conspiring neighbors, you will lash out and accuse, resulting in incarceration/being institutionalized.
