Beyond what >>527342424 said the whole Avatar thing was from an aborted plotline that Peter David was working up to in his 90's run. He basically wanted to kill Banner for good and have the Hulk entity play host to various other people while Rick tracks it down.
Partly in Marvel said no so he did a slight retcon where that was part of a doomed timeline where Banner goes insane with grief and gives up letting Hulk take over.

This is a guy whenever he becomes close with someone they die at some point and the rage and anguish become a fuel for him. It physically pains him to see bad things happen to people and it sends him into a blind rage, literally.
