Yes, you'll feel an ache in there because the muscles your balls are attached to become sore/strained from the extra weight of the cage. These muscles like the small stabilizers in the back become sore from use, you are essentially hanging extra weight on them. They aren't like big muscles that can take a work out, these muscles are delicate and become strained easily. When you feel this coming, take the cage off and go for a hot soak in the tub to ease the soreness. Stretching will help, lay back with something under your hips and lower back, like an exercise ball or a spineboard, or the arm of your couch. Take a day or two off wearing it when this happens, then go back to it when the soreness goes away. Eventually your muscles will get more used to the extra weight. Becoming aroused and straining against the cage can also exacerbate this as your balls become tighter in the cage and want to lift closer to your body when aroused. Understand that you are essentially working those muscles out, and because these muscles are delicate and used to just the light weight of your junk, even a small plastic device can put strain on them. I used to have this after a few days with just my plastic cage, after working more into it I can now wear a steel one without much issue, I even wear it to work for 12 hours a day. Don't force yourself to keep wearing, take breaks and ease more into it and eventually your muscles will adjust.
