I'll implement a damage source system similar to bukkit's implementation, I've been aware of this bug for months and never bothered to look into it thoroughly as it was regularly a half heart of damage 5% of the time when logging in.
Seems to be occurring (a lot) more often since the region file format change so it's obviously something to do with how fast chunks load in.
