its actually really easy to explain this. despite crediting himself as the game designer and only person involved in the creative process, Kojima's actual contribution to Death Stranding clearly ended at "game where you deliver things", which is why you could see him so visibly confused and annoyed at the terrible gameplay demo he showed at the Dorito Pope's event.
Kojima only cares about making movies. Death Stranding is a movie franchise to him. The fact that that movie also has to have a game attached to it is largely irrelevant to him. That's why you get a product where the movie has little to nothing to do with the actual game part and you at no point will ever actually see any of those characters from the movie while you're playing the game.
It's still fun tho
