He's definitely gotten fat with age but if you watch the show you'll sometimes see a glimpse of him in tighter fitting scrubs and you can see that he's got ridiculously severe kyphosis. He's a lot less of a fat shit than he looks in his usual getup. In the earlier seasons he looked fat but was actually quite normal.
