Fingers actually look weird in the original picture and they were even worse in the 4o-generated mangafications, I guess it was AI created/edited in the first place (I also can't seem to find a source for that picture using Google/Bing/Yandex reverse search). I ended up asking Sora to not reference the picture for the fingers and it seemed to work.
Normally I prefer to use the pictures as inspiration for a txt2img prompt, in part because artefacts are often more likely with img2img, but I couldn't get the composition quite right and I liked that one a lot.

The NPC is the one who keeps repeating the same short, repetitive and stock-like, phrases over and over and over again whenever they talk. They usually have no personality and little-to-no relevance. You fit perfectly this description.
