The difference between making an episode on NAMbLA and making an episode on mormonism or scientology is that a lot of normies are already annoyed by mormons or scientologists, so mocking them is funny and cathartic. Even if they don't know the full extent of what scientologists actually believe, your average American understands that scientology is a goofy cult - seeing it get exposed and made fun of is seeing the manifestation of a relatable sentiment.
Conversely, your average American has no idea that there are organized pro-pedophile movements. The humor in making fun of said movements comes moreso from making fun of "pedophilia" as an evil with NAMbLA as a representation of such, not from making jokes about something that everyone can relate to.
The cult-mocking equivalent of South Park making fun of NAMbLA would be South Park making fun of something like The Church of Euthanasia - a niche, unknown-to-normies organization that can be used to represent an abstract evil with ridiculous beliefs that are easy to laugh at. But Matt and Trey rarely make fun of niche movements - they concern themselves more with issues they have an active interest in, which typically includes controversial subjects from popular culture. So why would they make fun of something as niche as NAMbLA, unless they're interested in it?

If they came onto the show with a speech about why children should have sex, they'd be dismissed immediately. One could easily say that they hide their interests under the guise of irony because they want to normalize the idea of child sexuality to the masses. It's similar to how nazis will hide nazi rhetoric under irony so as to make nazi ideas easier to digest.
