Considering even her poem in that volume references him, and ends with a hopeful line about the heart, I'm convinced it wasn't accidental. Ulquiorra and Orihime have quite a few moments like that, like when she looks guilty after healing Ichigo as he stares her down, despite having done nothing wrong to feel that way (or maybe it was due to the slap? Only Kubo knows desu). If Unmasked is anything to go by, he probably regretted not developing their relationship more to show how Orihime impacted him before he died. With that perspective, it also explains her reactions, since at times she looks downright betrayed at him, which makes perfect sense if you believe she wanted to be his friend before abandoning that idea due to circumstances and his callousness.
