Did your instructor not explain this to you?

Fill out the paper form with your application details and get your two references to sign it.
Then go to shoppers drug mart and get a passport photo taken. Paste the photo onto the photo card included with your paperwork and fill out the info on the back. Get your photo guarantor to sign their portion of the application paperwork.
Then take all of the above + your written and practical test results, and stick it in the pre-adressed envelope to Miramachi that your instructor should have given you. Go back to shoppers and mail it out at the Canada Post counter.

Photocopy everything before you send it.
