I work on the science side and it's intentional, Data for progression through clinical trials is extremely stringent in order to prove drug efficacy and tolerance within mandated levels. There needs to be a paper trail for 'everything' done because humans are notably fickle and we need to make sure if a side effect happens because 'we' fucked up or otherwise; it's a huge fucking deal and the FDA does not fuck around.
