Nuke what? There are almost half a million Russian citizens living on Sakhalin Island.
Russia would move forces and start destroying as many air and naval assets of Japan as they could. There are also military bases on Sakhalin that would work to resist the invaders.
This is kind of a dumb question, although I personally do wish Russia would finally settle the land issue with Japan in one way or another.
