The better question would it be the setting. In modern times with the changes being the result of some fad diet or experimental medication? A quasi-medieval fantasy where a noble with a breeding kink has a concubine mutated with magic? A sci-fi setting where space colonization creates a scenario where underpopulation is a serious issue?
