But actually, Isaac has been dead for a very long time. Every run is him, being stuck in a Purgatorium kind of place, him wandering an endless nothing, for he is lost and all he has is his past now.
That's why The Forgotten is a character, that you unlock by unearthing his bones. That is Isaac's body...
All of the game is his memories, his life, and him wandering it all, again and again.
In the Mother ending, he finally finds his way, and in the true ending, he finally sheds all guilt, all his pain, all his fear.
...He becomes lighter, and his weary soul finally finds rest.

Yes smiley anon hahah there are. Each ending after number 11 just tells you a new bit of the story, though, like another chapter of a novel, culminating in the "True" ending.
