I knew a guy that blew off half his foot with a blank, so yes it can be very harmful, even if there's no projectile.

The guy I knew was working at a wild west theme park as a stunt man and accidentally fired his 10ga coach gun into his foot when he fell off a roof.
To his credit, the guy walked it off, and performed the rest of the stunt show before seeking medical attention, he didn't want to frighten all the children watching the show.
That happened like 30 years ago, but apparently they still do these live stunt shows. Pic rel.
