Okay so my main mod is Hospital, which randomly sends patients via drop pod with mild injuries and sometimes sends mass casualty events of 15-20 pawns. This is the backbone of it. You get good money for doing this.
The second part is the scenario, pic related. Boost to medical and a lot of resources to start so you can very quickly build a working hospital. Setting all factions to a slightly positive relationship makes them less likely to attack you (but this increases the frequency of mechanoid raids I think). The scenario incidents drop ship chunks every 1-3 days, ship parts every 10-30 days, a transport pod crash every 3-5 days, a shuttle crash every 15-30 days, a resource pod crash every 1-3 days, faction assault every 15-30 days, and a space battle every 20-30 days. The theme of the mod is that a war is going on and you're basically the red cross.
Now the other mods are where it gets interesting. Factional War causes two factions to show up on your map and attack each other, leaving you as a bystander as long as you don't piss off one of them.
There are other mods in place like Dubs Rimkit for medikits, Stabilize Bleeding for emergencies, the medical module for Vanilla Expanded to expand the hospital's capabilities, autopsy table as an organ source, and Casual Surgery Rebalance to prevent the bullshit that is Rimworld's "Oops I lobotomized the guy instead of adding a pegleg".
One thing I found out the hard way is that when warring factions decide to clean up the enemies this includes people in your care, so I recommend getting a nonlethal weapon mod like tranquilizer guns so you can arrest them and not harm faction relations.
