Condoms help but don't cover every area that STIs can be transmitted from.
STIs are mostly spread through body fluids, so you've got a chance of catching them through cum, residual fluids around the genitals/anus, or even saliva. Not to say you're guaranteed to catch AIDS the second you have gay sex, as the chance of catching varies by what exactly you do (top/bottom, positions, just BJs or full anal etc) but in general it is better to take PREP just to be on the safe side.
If not on PREP it's also recommended to get tested every 6 months or so if you're regularly having sex with different partners.
