The improved skin texture and pigmentation is almost certainly *mostly* from phototherapy, maybe also retinoids and chemical peels; surgery doesn't do that, and it looks too dramatic to be *just* retinoids and chemical peels.

She does look to have had a nose job, but I'm skeptical that anything else was done surgically.
