The only things Okarun has killed are aliens and yokai (already dead). He had a chance to kill Hase, the only normal human he's fought, and he didn't, instead giving him over to medics. He also is shown to be generally self-sacrificing, offering to sever his dick to give to Mantis Shrimp for a temporary boost to his funds so he could treat Chiquitita. He's a good guy.
