Just be straight up and tell him what you're worried about and what's on your mind. Be polite and respectful but at the same time real and transparent. Start off asking for forgiveness for your sins to sort of cleanse yourself before praying. For example

Then the Lord's Prayer. Basically the meat and potatoes of it is you tell him what's going on in your life and what you want to accomplish. I think he honestly enjoys listening and dislikes people hiding things and being dishonest like Adam and Eve did.
