They don't have complete omnipotent control over everything at all times in their realm. Think of it more like them creating a house for themselves. There can still be mice running around in the walls of a house you built. Portals can be opened into your realm without your knowledge, someone can do something quickly or stealthily, and then leave.

This is actually what Meridia has been doing to Molag for a long time in canon, so it's fun to see a mod author doing the same thing. Meridia crashed an entire city into Coldharbour and put a bubble around it, and Molag knows they're there but can't do anything about it without expending effort and resources, which at the time Meridia did this he was busy invading tamriel so they got to stay.

Additionally, even before that there were rebel vampires that were unwillingly infected that always resented him and undermined him in Coldharbor, and then there are also multiple cases of entering and leaving Gloomshade without permission as well.
