Shigesato Itoi has always been a man of normalcy. The Magypsies for example were a natural inclusion as trannies exist in the real world as well, but Itoi never shied away from the fact that they're ridiculous. It is more grounded in a sense. Just as you might think the transexual accountant at the reception is kind it's still a man in a dress, and real people can distinguish and find it entertaining without trying to pretend something isn't what it is.

Undertale on the other hand goes out of its way to falsify reality. It's exactly like the homestuck fanbase - In reality a bunch of overweight trannies but posing as thin dark edgy cool kids.

If SHigesato Itoi made Undertale he would make sure that every character was just what they are - socially awkward acne-ridden shut-ins.
