I'm pretty sure it's been implied there's other parts of the realms where it does other shit and we're only interacting with the stuff the corner the game is tucked into. Like I think "Hooked on You" is technically canon and still in the realms.

It feeds on both negative and positive emotions so there could very well be paradise realms full of orgies and cake and cake orgies too.

What we see could just be the super unlucky ones.
