they probably have them but then again, healthcare in the imperium goes from non existant to the absolute bare minimum for the majority of humans (hivers) which would be closer to combat medic job of patching you to get back to work ASAP because a human life is a cog in the machine and just so happen to be the cheapest thing on the block.
If you go up the social (and economic) ladder then it's probably expensive because you probably need a doctor or a magos biologis with the expertise, with the mechanicus-sanctionned gear for it.
In the case that you are a rich guy banging his navigatrix, you wouldn't need IVF unless one of her (early, let's be real you're not banging the older ones with full on Dune spice melange mutations, not because you won't but because the family doesn't want you seeing them) mutations somehow affect her reproductive organs/cycles.

Great Crusade/Horus Heresy era had better tech, even if it still perplexes me how all the colonies of the imperium were literally exploited as much as possible (both material and bodies, big reason half of them went "fuck you I'm joining Horus") and most of the tech was clearly targeted toward anything military-oriented (you know, galaxy wide conquering and all that) that they still had the time to have civilian stuff.
