In the art book it is implied that CENSORED's look is constantly changing and is basically personalized based off what the viewer has went through in life. Essentially, it’s a reminder for someone of something incredibly traumatic they might have witnessed, but twisted and warped in such horrifying ways it drives them insane just by viewing it.

And that's basically what Gregor's arm already is.
